Notice: Before you can begin trading, you must
complete the following procedure:
- You MUST have an existing live account with CMS International to use
this procedure. If not, click
here to follow the standard account opening methods.
- Select an appropriate account application form from the table
below.
- Fill out and sign the applicable pages.
- Fax/Mail/Email the above pages to us.
- Immediately notify us that you have sent the documents by
emailing customerservice@cmsfx.bm.
- Funds should be sent ONLY AFTER APPROVAL of
your account.
IMPORTANT: Only the following types
of deposits are acceptable:
All other types of payment will be returned at the owner's
expense!

|
NOTICE:
Joint accounts between an individual and a corporation, an introducing
broker and any client, or two corporations, are not permitted.
Download
the joint account application form. Each account holder must fill out
separate copies of the first page, and sign the disclosure. For clients
introduced by an Introducing Broker (IB), enter the IB's name in the
"Disclosure for Clients Introduced to CMS International by Third Parties". Items to
fax are: separate copies of the first page for both individuals, one
disclosure page with both signatures, and a filled-out IB disclosure,
if necessary. |

|
NOTICE: Joint accounts between two corporations are not
permitted.
Download the corporate account application form, and fill out pages 1, 3, and 4.
If you authorize a third party to trade on your behalf, fill out the
"Limited Power of Attorney" as well.

Download the managed account application form.
Fill out all applicable pages. Money Manager: Requires Name &
Signature of Trading Agent/Joint account holder; Requires Client
Signature on "Limited Power of Attorney". Your Money Manager must also
submit copies of two government issued photo identification documents.
